NRS notification webhook (no auth; always 200)

NRS (Convoy) posts {irn, message}. Always 200, also to malformed input; idempotent on (irn, message); never trusted: the IRN is re-read from NRS with the operator's Access Point key before anything changes. The operator is resolved from the request host (X-Forwarded-Host, then Host) or ?operator=<org slug>.
